Clinical History

Sudden severe headache, worst-of-life, with brief loss of consciousness.

Technique

Non-contrast axial CT images of the brain were acquired.

Comparison

No prior imaging available.

Findings

Acute hyperdense subarachnoid hemorrhage layering along the right Sylvian fissure and basal cisterns, Fisher grade 3. No midline shift. Ventricles are normal in size.

Impression

Acute subarachnoid hemorrhage, right Sylvian fissure — recommend urgent neurosurgical/neurology consultation and CT angiography to evaluate for aneurysm.

Recommendations

Urgent CT angiography brain; neurosurgery referral.
